Properly fill out bank deposit slip in duplicate, please indicate the following: |

Type of Deposit (Indicate whether the deposited amount is in cash or check. Please be advised that check deposits are subject to bank clearing. Your Control No. & PIN Password will only be transmitted once your check has cleared); |

The deposited amount (Please write the amount clearly in words & figures); |

|
For check payments, please indicate the drawee bank & the check number; |
| |

|
Please write your name or company , your User ID, & your iNTOUCH VoIP phone number so as to properly identify your end user/account subscription details. |

Please make certain that the bank teller has properly stamped & received your deposit; |

Once we have verified your deposit & that everything is in order, we will immediately transmit via electronic mail (Email) your Control No. & PIN Password; |
